INTERNET-BASED INFORMED CONSENT SCRIPT

Title of Research Study: Entrepreneur COVID-19 and the Well-Being Study

Thank you for your interest in this research project. This research study will examine the psycho-entrepreneurial impact of the mandatory transition to remote business attributable to the COVID-19. I would like to ask you a few questions to determine whether you are eligible to participate in this research.

The screening will take about  2 minutes of your time>. I will ask you some questions about your status as an entrepreneur. You do not have to answer any questions you do not wish to answer or are uncomfortable answering, and you may stop at any time. Your participation in the screening is voluntary. 

We will make our best efforts to keep your answers confidential. No one except for the research team will have access to your answers.  If you do not qualify for the study all of your answers will be destroyed.  If you qualify for the study and consent to participate in the study your responses to screening questions will be kept confidential and will only be reported in summary form.
